Benefit Updates - What’s New: NEW DISABILITY CARRIER! Disability is moving to a new carrier. This coverage pays you a portion of your earnings if you cannot work due to sickness, injury or pregnancy. • You can purchase up to $7,500 of coverage, not to exceed 66 2/3 % of your current monthly earnings. • Elimination Periods: 0/7, 14/14, 30/30, 60/60, 90/90, 180/180 • For elimination period of 30 days or less, if you are confined to a hospital for 24 hours or more due to a disability, the elimination period will be waived, and benefits will be payable from the first day of hospitalization. • If your disability starts with a pre‐existing condition, your policy will pay benefits for a maximum of 4 weeks. • If you were insured under the prior disability plan and not receiving benefits the day before this policy is effective, there will not be a loss in coverage and you will get credit for your prior coverage.

SICK LEAVE BANK UPDATES The purpose of Sick Leave Bank (SLB) is to provide additional paid sick leave days for members who have exhausted all available paid leave in the event of the catastrophic illness or injury of the employee or the employee’s immediate family member. • Employee must be able to donate two (2) local leave days to become a member • Application for membership will become active when the two (2) days are earned. Section 125 Cafeteria Plan Guidelines A Cafeteria plan enables you to save money by using pre-tax dollars to pay for eligible group insurance premiums sponsored and offered by your employer. Enrollment is automatic unless you decline this benefit. Elections made during annual enrollment will become effective on the plan effective date and will remain in effect during the entire plan year.

Changes in benefit elections can occur only if you experience a qualifying event. You must present proof of a qualifying event to your Benefit Office within 30 days of your qualifying event and meet with your Benefit Office to complete and sign the necessary paperwork in order to make a benefit election change. Benefit changes must be consistent with the qualifying event.

A change in marital status includes marriage, death of a spouse, divorce or annulment (legal separation is not recognized in all states).

FSA/HSA Limitations: Please note, in general, per IRS regulations, married couples may not enroll in both a Flexible Spending Account (FSA) and a Health Savings Account (HSA). If your spouse is covered under an FSA that reimburses for medical expenses then you and your spouse are not HSA eligible, even if you would not use your spouse's FSA to reimburse your expenses. However, there are some exceptions to the general limitation regarding specific types of FSAs. To obtain more information on whether you can enroll in a specific type of FSA or HSA as a married couple, please reach out to the FSA and/or HSA provider prior to enrolling or reach out to your tax advisor for further guidance.
